Our free, no obligation Home Care Consultation gives you an opportunity to learn more about Trail Ridge Home Care. You can ask any questions, learn more about our services, and become comfortable with our organization. In the initial meeting we encourage you to talk in-depth about the needs of your loved one and any concerns you may have. We understand inviting Trail Ridge Home Care into their/your life is a big step and we want to make sure everyone involved is understood and supported.
During the Care Consultation we take the time to become familiar with the needs, lifestyle and daily routines of your loved one. Our experience in senior care enables us to recognize areas within the home or daily activities that may pose a risk to your loved one. We will identify responsibilities our Caregivers will take over to ensure your loved one is safe, and that their home is well taken care of.
Along with this lifestyle information we also gather basic medical information. For example:
- Care physician(s) contact information
- Current therapeutic needs
- Primary health concerns
- Physician recommended plans
- Level of mobility
- Emergency contact information

Using this information we will develop an individual care plan and schedule that works for you.
